- Download and Print the “Health Release and Parental Notification Form.” (Make sure it is completely filled out, signed and notarized).

You are responsible to download the what-to-bring list and to arrive on Monday, July 28 with all of the required items. You must arrive wearing blue jeans and a white T-shirt.
